If a person needs an IT account who is not a current student or employee, the sponsor will need to request an accountshould send this form to the person requesting. Ask them to fill in your information as their sponsor. Please provide them with the guideline links for review as well. Any questions please submit a request via the MIS Service Desk at sc.sage.edu.
