How to Request an IT Account for non-employees

If a person needs an IT account who is not a current student or employee, the sponsor should send this form to the person requesting them to complete. They should use your information as their sponsor. Any questions please submit a request via the MIS Service Desk at sc.sage.edu.

By filling out the form, they agree that they will consent to the guidelines under the "Guidelines" section below.
